Cities in Monsilva are designated by the government usually depending on their population and urbanization. According to the government, for a town to be designated as a city, it must have a population of over 77,000 people and clearly show common city characteristics, such as commercial and residential areas, a road system and a clear non-agricultural purpose. Once a town is given city designation, the Monsilvan word shì (市, "city") is added to their name. There are three cities in Monsilva, known as 'honorary cities' that are designated as cities but do not meet the population requirements, these are Wen'an and Lunhuang in Weishi and Rongmei in Shaoyu. They are designated as honorary cities due to their historical importance in the country and, in the case of the two Weishi cities, due to their large tourism industries.

In terms of administrative divisions, a city is represented by one or more subdistricts. There are three types of city designation: government cities, autonomous cities, and county cities.

Government cities are the largest cities in the country. They are made up of multiple counties and subdistricts, which are often governed by a city-wide council instead of individual county councils. Government cities are made up of counties designated 'constituent urban'. Government cities that have counties with their own councils will still have a city-wide council, and the county councils are subordinate to the city-wide council. They are often very large, consisting of multiple urban areas that could be considered individual cities in their own right.

Autonomous cities are cities within a county that are administered independently of the county's government. These cities take up a portion of a county's subdistricts and are governed by their own city councils. Although these cities are part of the county de jure, de facto they behave as seperate counties. This was often a contentious issue when counties were used for federal elections, however this system was abolished in 2023 and since then autonomous cities have had significantly increased representation in parliament. Some counties can contain multiple autonomous cities. Counties that contain autonomous cities are designated as 'standard urban' counties.

A county city is a city that is administered by the county council. County cities are made up of all of a county's subdistricts. For some county cities, the council is still called a city council. Counties which contain a county city are given the 'city urban' designation.

Monsilva has a total of 73 cities: 17 being government cities, 39 being autonomous cities and 17 being county cities. Around 43 million people, 74.1% of the overall population, live in these cities.
